조대술에 의한 하마종의 치험례

Abstract
서론
하마종은 설하선 분비도관의 파열로 인하여 타액이 분비도관에서 빠져나와 주위의 연조직
내에 저류됨으로써 구강저에 발생하는 가성낭종으로서, 병소의 내벽이 상피로 피복되어 있
는 경우는 매우 드물고, 대부분의 경우 압축된 결체조직으로 피복되어 있다.
하마종은 동통등 주관적인 증상은 별로 나타나지 않으며 감염되는 일도 극히 드물다고 알
려져 있지만, 병소의 크기가 증가함에 따라 혀가 거상되어 그 움직임에 장애를 받게 되므로,
저작과 발음을 원활히 수행할 수 없게 된다. 적절하지 못한 하마종의 치료는 병소의 재발을
야기시키게 되는 데, 재발이 잦은 하마종은 환자에게 정신적으로도 해로운 영향을 미치게
된다. 더구나 악설골근 하방의 인접공극까지 확산되는 plunging형의 하마종의 경우는 치료
를 매우 어렵게 만들며 결국 설하선까지 희생시켜야만 된다.
하마종의 치료법중 단순절개배농법은 거의 모든 경우에서 재발된다고 보고되고 있으며, 하
마종의 낭강과 구강점막 사이에 영구적인 누공을 얻고자 하는 여러가지 방법중 현재에는 조
대술이 가장 효과적인 방법으로 인정되고 있다. 방사선사용법은 plungin 형 하마종의 치료
법으로서 그 효과를 인정받고 있으며, 재발을 근본적으로 방지하기 위해서는 하마종과 연관
된 설하선까지 완전 절제해야 한다.

Ranula is a thin-walled, bluish, transparent cyst which in effect is a mucocele ocurring
in the floor of the mouth beneath the tongue. It is a nonpainful lesion, but the tongue
may be raised and its motion hindered, thus impairing mastication and speech.
Two girls (a 7-year-old and a 12-year-old girl) visited the department for the
management of soft tissue swelling in the floor of the mouth. Clinically, they showed
typical features of the ranula.
They were treated by marsupialization which is generally agreed on as the best
surgical procedure for the ranula. The procedure consists of excising the superior wall
of the ranula and suturing the cystic lining to the mucous membrane of the floor of the
mouth to make it continuous with the oral cavity.
The marsupialisation is accompanied by a high rate of recurrence. Because the ranula
itself is lined by compressed connective tissue rather than epithelium. Generally, it is
sufficient to excise only the overlying mucosa and submucosa to gain access to the
sublingual gland and to eliminate any tissue redundancy after surgery.
