Ranula is a thin-walled, bluish, transparent cyst which in effect is a mucocele ocurring
in the floor of the mouth beneath the tongue. It is a nonpainful lesion, but the tongue
may be raised and its motion hindered, thus impairing mastication and speech.
Two girls (a 7-year-old and a 12-year-old girl) visited the department for the
management of soft tissue swelling in the floor of the mouth. Clinically, they showed
typical features of the ranula.
They were treated by marsupialization which is generally agreed on as the best
surgical procedure for the ranula. The procedure consists of excising the superior wall
of the ranula and suturing the cystic lining to the mucous membrane of the floor of the
mouth to make it continuous with the oral cavity.
The marsupialisation is accompanied by a high rate of recurrence. Because the ranula
itself is lined by compressed connective tissue rather than epithelium. Generally, it is
sufficient to excise only the overlying mucosa and submucosa to gain access to the
sublingual gland and to eliminate any tissue redundancy after surgery.
